FABL will then forward the original Continuing Education Credit Report with a copy of your online evaluation form and a copy of your course certificate of completion to the State.
We will also forward you the original course certificate of completion, and copies of your online evaluation form and the Continuing Education Credit Report for your files.
Remember, you must send us the original Continuing Education Credit Report with your original signature and we must forward the original with our signature to the State in order for you to get credit.
